Written care instructions: note what you understood and what still needs clarification

Keep the original instructions

MedlinePlus encourages asking questions and requesting written instructions when needed. Retain the document actually provided, including its date and service name. Your personal notes should sit beside it rather than replace it with a rewritten version that may omit qualifications.

Make a clarification column

For any unclear term or action, write a question and leave space for the professional’s answer. Distinguish the original instruction, your understanding and the clarification you later received. A family helper should not silently turn a guessed meaning into a confirmed instruction.

Confirm the communication route

Ask the service how clarification should be requested and which channel is appropriate. Do not assume a public comment box or an unrelated website can provide it. This organisational method is not advice to modify care: clinical instructions, prescriptions and urgent concerns belong with the responsible professional and applicable local services.
