A vaccination record is evidence of an event, not a new schedule to prescribe

Identify the event record

A record should distinguish an administration document from a reminder or an appointment booking. Keep the issuer and the exact fields available rather than reconstructing details from a photograph of a clinic sign or a remembered visit.

Preserve the original and the transcription

An organisational summary can include the date and vaccine identity as stated in the original. Mark any missing product or dose information as unconfirmed. Do not fill a blank from a relative’s record or infer that a familiar brand name identifies every detail of the event.

Keep future decisions separate

This article does not create a vaccination timetable, decide eligibility or recommend repeating an uncertain dose. The correct next step depends on the applicable guidance and professional review of the actual history. MedlinePlus’s guidance on discussing and accessing records is an educational starting point; this site does not issue or authenticate vaccination certificates.
